Your report helps the office understand unresolved concerns, recognize recurring issues, and consider appropriate follow-up or referral.
How to raise a concern now.
- Contact the Corporate Office. Call (501) 231-7697 or email the Association using the contact information below. The website’s online submission connection is not active.
- Describe the facts and previous steps. Give dates, the affected service, whom you contacted and what response you received. Ask the office how to share sensitive records appropriately.
- Confirm receipt and follow-up. Ask who is handling your concern and when to check back. Do not assume information has reached the office without acknowledgment. A response time or outcome is not guaranteed by this page.
